How My Mind Lab collects, protects, and handles your clinical, educational, and developmental data in compliance with Australian standard privacy regulations.

1. Introduction
At My Mind Lab, we are committed to protecting your privacy, maintaining confidentiality, and providing psychological services with professionalism, integrity, and respect.
This document explains how we collect, use, store, and disclose your personal and health information, as well as the consent process for psychological assessment, therapy, and related services.
We comply with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and applicable professional and ethical standards for psychological practice.
2. Information We Collect
To provide psychological assessment, diagnostic, and therapeutic services, we collect personal information and sensitive health information that is relevant to your care. This may include:
Client and Guardian Information

Name, date of birth, contact details, address, emergency contact details, and, where applicable, parent or guardian information.
Health and Clinical Information

Medical, developmental, psychological, educational, and family history.

Current medications and details of treating health professionals.

Previous assessment reports, diagnoses, and relevant clinical records.
Assessment Information

Information obtained through interviews, questionnaires, behavioural observations, cognitive and psychological assessments, teacher questionnaires, school reports, and other relevant documentation.
Administrative and Funding Information

Medicare, private health insurance, NDIS details, referral information, appointment records, and payment information where applicable.
3. Why We Collect & Use Your Information
We collect, use, and store your personal and health information only where necessary to provide safe, ethical, and effective psychological services. Your information may be used to:

Conduct comprehensive, evidence-based psychological assessments, including assessments for Autism, ADHD, AuDHD, learning differences, intellectual developmental differences, cognitive functioning, and other developmental or mental health concerns.

Prepare psychological reports and recommendations for you, your family, medical practitioners, schools, or funding bodies such as the NDIS, with your consent where required.

Plan, coordinate, and deliver psychological therapy, parent coaching, and early intervention services for children and families.

Communicate with other health professionals and service providers involved in your care, where you have provided consent.

Manage appointments, billing, Medicare and NDIS administration, and other practice-related functions.

Meet our legal, professional, and regulatory obligations.
4. Confidentiality & Disclosure Limits
We understand that the information you share with us is personal and confidential. We are committed to protecting your privacy and maintaining confidentiality in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and applicable professional and ethical obligations for psychological practice.
Your personal and health information will only be disclosed:

With your informed consent.

Where necessary to coordinate your care with other health professionals, educational providers, or support services, with your consent.

Where there is a serious and imminent risk to your safety or the safety of another person.

Where mandatory reporting obligations apply, including concerns relating to child protection.

Where required or authorised by law, including legal obligations such as court orders or mandatory reporting requirements.

Your psychologist may participate in professional supervision or peer consultation to support safe and effective care. Where possible, identifying information is removed to protect your privacy.
5. Storage, Security, & Records Retention
We take reasonable steps to protect your personal and health information from misuse, interference, loss, unauthorised access, modification, or disclosure.
Electronic records are stored securely using encrypted, password-protected clinical practice management systems. Paper records, where applicable, are stored in secure locations and are accessible only to authorised personnel.
We retain clinical records in accordance with Australian and Queensland legal requirements. Generally:

Adult client records are retained for a minimum of seven (7) years from the date of the last clinical contact.

For clients who were under 18 years of age at the time of their last clinical contact, records are generally retained until the client reaches 25 years of age, unless a longer retention period is required by law.
6. Access to & Correction of Personal Information
You have the right to request access to the personal information we hold about you and to request corrections if you believe the information is inaccurate, incomplete, or out of date.
Requests should be made in writing and will be considered in accordance with the Privacy Act 1988 (Cth) and applicable privacy legislation.
In limited circumstances permitted by law, access may be refused or provided in an alternative manner, such as through your treating psychologist, where releasing the information may pose a serious risk to the health or safety of you or another individual.
