Terms of Service &
Clincial Agreement
Please read these terms carefully before engaging and booking services with My Mind Lab. By scheduling assessments or therapy sessions with us, you agree to comply with these terms.

1. Practice Scope and Eligibility
My Mind Lab is a professional psychology practice providing specialized diagnostic evaluations, neurodevelopmental assessments, and targeted therapies. Our assessment services—spanning cognitive, learning difficulties, intellectual developmental disorders, Autism, ADHD, and AuDHD (combined presentation)—are available to individuals of all ages. Diagnostic assessments and comprehensive report deliverables are provided for self-understanding, schools, medical practitioners, and the NDIS, and are available both online (telehealth) and offline (in-person) in Queensland (QLD). Interactive and family-focused therapy services for young autistic children are selectively offered in Logan and Ipswich, QLD.
2. Not an Emergency or Crisis Service
My Mind Lab operates strictly as an outpatient diagnostic and therapeutic clinical service. We do not provide urgent mental health assessments, crisis management, or emergency intervention. If you or a family member are in immediate distress or require urgent support, please clear this page and immediately contact emergency services by dialling 000 (Australia), visiting your nearest hospital emergency department, or contacting Lifeline on 13 11 14.
3. Booking, Payment, and Fee Schedule
By scheduling an appointment (including assessment sessions, clinical interviews, or therapy consultations), you agree to our standard billing terms:

Assessment Intake Fees: A deposit or upfront payment schedule may be requested at the time of booking to secure your assessment block.

Session Payment: Fees are due and payable either at the time of booking or on the day of the scheduled session unless direct billing is formally pre-approved (e.g., plan-managed NDIS clients).

Report Release Hold: Consistent with Australian private clinical practices, drafts or final diagnostic reports will not be released to yourself, educational institutions, medical practitioners, or the NDIS until all outstanding session and reporting fees are paid in full.

4. Cancellation and Rescheduling Policy
To maintain care accessibility and offset reserved practitioner hours, My Mind Lab adheres strictly to professional practice cancellation guidelines. When you book a session, we set aside professional clinical hours specifically for you. If you need to cancel or shift your session, please notify us as early as possible. Safe practice cancellation policies include:

0 to 24 Hours Notice: Cancellations within 24 hours of the scheduled time attract a late-cancellation fee equal to 100% of the session rate.

24 to 48 Hours Notice: Cancellations within 24 to 48 hours of the scheduled session attract a late-cancellation fee equal to 50% of the session rate.

More than 48 Hours Notice: No cancellation fee will be charged, and your deposit can be fully applied to a rescheduled date.

NDIS Cancellation Rules: For NDIS participants, late cancellations will be billed in accordance with the current NDIS Price Guide limits.
5. Technical Requirements for Telehealth (Online Assessments)
We take reasonable steps to protect your personal and health information from misuse, interference, loss, unauthorised access, modification, or disclosure.
Electronic records are stored securely using encrypted, password-protected clinical practice management systems. Paper records, where applicable, are stored in secure locations and are accessible only to authorised personnel.
We retain clinical records in accordance with Australian and Queensland legal requirements. Generally:

Adult client records are retained for a minimum of seven (7) years from the date of the last clinical contact.

For clients who were under 18 years of age at the time of their last clinical contact, records are generally retained until the client reaches 25 years of age, unless a longer retention period is required by law.
6. Report Deliverables & Administrative Limits
Our comprehensive diagnostic reports represent a synthesis of historical intake data, standardized clinical tests, and expert observation. Our standard timeline for final report delivery is 3 to 4 business weeks following your final clinical testing session. While we specialize in providing clinical evidence matching the operational criteria for the NDIS, school modifications, or medical treatment plans, clinical outcomes are purely diagnostic. My Mind Lab cannot guarantee external funding approvals, specific NDIS support package tiers, or academic accommodation decisions by third-party bodies.

6. Limitation of Liability & Clinical Consent
For young children being assessed, we require full, joint parent/guardian consent in situations where shared parental responsibility orders exist. Clients are legally obligated to provide true, current, and complete custody, developmental, and medical documentation. My Mind Lab, including its clinical staff and supervisors, shall not be held liable for any decisions, damages, or diagnostic discrepancies resulting from inaccurate or withheld client background information.
